New for the 2022-2023 school year: “Healthy Roster” electronic medical records.

For sports medicine resources, questions about Healthy Roster, and how to download the Healthy Roster App please visit the sports medicine page on the school website located under the athletics tab.

Physicals
All physicals must be electronically uploaded to Healthy Roster.

All students must establish a Healthy Roster account by emailing csiebens@msjnet.edu or dyoung@msjnet.edu with their name, birthdate, and graduation year.  A physical dated after May 23, 2022 is required to be uploaded into this profile before they will be allowed to try out for a team, along with filling out and signing the Acknowledgement and Assumption of Risk and Emergency Contact and Travel Permission forms located under the documents tab of the athlete’s profile.  Video tutorials, resources, and explanation of Healthy Roster required documents/features can be found on the sports medicine page of our school website: https://www.msjnet.edu/athletics/sports-medicine

If your insurance only allows a physical every two years, or you are not due for a new physical, we accept Patient First or Minute Clinic physicals. This will satisfy the requirements for participation in athletics.

It is recommended by ImPACT that you use Google Chrome to access the test and allow pop-up windows for the duration of the test. Complete the test in a distraction-free environment.
