Physicals
All students must have a current physical examination on file for the 2019-2020 school year before they will be allowed to try-out for a team. The completed physical form should be on file with the school nurse prior to the first day of try-outs, or your son may turn in the completed form to Ms. Michele Connor, Asst. Athletic Director or Mr. Kraig Loovis, Athletic Director on the first day of try-outs. Forms can be found on the website under athletic/forms.

If your insurance only allows a physical every 2 years, or if you are not due for a new physical prior to the start of the 2019/2020 winter season we do accept Patient First or Minute Clinic physicals. This will cover the requirements for participation in athletics at the Mount.
