Important information regarding physicals, sportsmanship meetings, and fall sports tryouts.
Physicals
MSJ athletics requires that all physicals be electronically uploaded to HealthyRoster.
Anyone trying out for a sport must have a valid physical uploaded to HealthyRoster prior to August 1st. All students are required to have a physical dated after May 17, 2026 and submitted to HealthyRoster. A physical is valid for one year from the exam date. The form is available on the Mount St. Joseph website under Parents- Health Procedures & Forms and on the Sports Medicine page under Referrals.
Please note that this does NOT apply to the school nurse’s SNAP Portal or vice versa. You MUST upload separate required documents to HealthyRoster and SNAP.
New MSJ Athletes: Under the Sports Medicine page of the MSJ website, locate the “HealthyRoster” dropdown and follow the instructions. You will fill out the jotform and will receive an access code from the Athletic Training staff within 24-48 hours. Using the access code, you will gain access to the athlete’s Healthy Roster profile.
Returning MSJ Athletes: Log in to your HealthyRoster and navigate to the Documents tab. Open the documents under “Expired Documents” and you will be prompted to fill out a new form.
Once you set up the Athlete profile, please fill out all forms found under “Documents." This will include:
-
Acknowledgement and Assumption Risk: Read completely and parent/athlete both initial/sign in all relevant fields
-
Emergency Contact and Travel Permission: Fill out all fields completely (including insurance info) and sign with your finger or a stylus in the gray box
-
Pre-participation physicals with physician signatures should be filled out by hand on paper form and scanned in using the icon in the top right corner/your phone’s camera
Please note that MSJ accepts physicals from Patient First, Minute Clinic, and other walk-in health clinics.
ImPACT Testing is no longer a requirement for participation in MSJ athletics. If you have any questions feel free to contact athletic trainers Damon Young or Amir Defino.
