Physical Forms

Freshmen and transfer students must have a current physical and immunization record on file prior to tryouts.  A packet containing the school’s health forms will be mailed out to freshmen in April but can also be downloaded on our website.  Transfer students will receive their forms from the Admissions Office.

Please note that we have a new policy regarding our physicals for 2010-2011.  All new students are required to have a physical between June 1, 2010 and August 1, 2010.  Please mail your completed physicals to: Mount Saint Joseph High School, Attention: Health Suite, 4430 Frederick Avenue, Baltimore, MD 21229. 

If participating in a fall sport, the physical can be given before tryouts to: Mr. Paul Triplett, Athletic Director, Mr. Jon Glover, Athletic Trainer or Mrs. Michele Connor, Assistant to the Athletic Director. ***It’s also recommended to make a copy of your son’s health forms for your own file.

Returning students must also have a current physical on file to tryout for a sport.  A physical is considered current if it is less than 1 year old and does not expire while you are playing a sport.  If the physical is done without our form, the physical must include a recent blood pressure, pulse, height, weight, and clearance for sports from a physician or licensed nurse practitioner.

*Kernan Sports Medicine will be offering high school physicals to our students this summer on 3 different days in June, July, and August. If your insurance will only cover a physical every 2 years, this is a great way to meet your requirements for participation in athletics at the Mount.
